民國75年10月31日為蔣中正總統百歲誕辰，由徐生水自製並贈送中正紀念堂。外以木盒裝放，盒上以金漆書寫「永懷領袖」，上款「蔣中正總統百年誕辰紀念」下款為「中華民國七十五年徐生水恭製」。紀念牌材質為紅銅鍍金，圓形，正面刻有蔣中正總統半身像，上刻「蔣中正總統百年誕辰紀念，中華民國七十五年」，反面刻有全中國地圖，地圖中有「三民主義」書冊，寓意以三民主義統一中國。物件固定於木製花紋框架，木架上並有「中正紀念堂惠存」的字樣。

On October 31, 1986, the centenary birthday of Chiang Kai-shek, Xu Shengshui made and donated this commemorative plaque to CKS Memorial Hall. The commemorative plaque is placed in a box inscribed with golden words "In everlasting memory of the late president Chiang," with a dedication "Chiang-Kai-shek 100-years-old Memory," and the lower part, "Respectfully made by, Xu Shengshui, 1986." The commemorative plaque is made of gilded red bronze, round, with President Chiang's bust portrait engraved on the obverse and encircled by inscriptions "Chiang-Kai-shek 100-years-old Memory, 1986" and on the reverse, a map of China and a book of "Three Principles of the People," for implying China's reunification under Three Principles of the People. The plaque is fixed on a wooden stand decorated with floral patterns and inscribed with the words "To Chiang Kai-shek Memorial Hall."
