民國75年10月31日為蔣中正總統百歲誕辰，由徐生水自製並贈送中正紀念堂。墨綠色玉牌正中為蔣中正總統像，周圍以「蔣中正總統百年誕辰紀念，中華民國七十五年」刻字圍成圓圈，玉牌鑲嵌於木製花紋框架中，木架上以金漆書「中正紀念堂惠存，徐生水恭製敬獻」的字樣。

On October 31, 1986, the centenary birthday of Chiang Kai-shek, Xu Shengshui made and donated this commemorative plaque to CKS Memorial Hall. The dark green jade plaque is engraved with Chiang Kai-shek's image encircled by inscriptions "Chiang-Kai-shek 100-years-old Memory, 1986." The jade plaque is embedded in a wooden stand decorated with patterns and inscribed with the words in golden paint "To Chiang Kai-shek Memorial Hall, Respectfully, Xu Shengshui."
